To schedule an initial appointment, you may contact me by telephone at (415) 484-5210 or by email at dr.valerie.crawford (at) gmail.com or use the Contact form on this website.
Fees and services
Please contact me by phone to discuss fees for my various services. For couple therapy, I typically use 75 or 90 minute sessions. For EMDR, I highly recommend 75 minutes, with one to three 2- or 1.5-hour sessions toward the beginning of treatment.
Payment methods
I accept payment by credit card via a client portal account established for each client.
Insurance
Insurance companies often require significant disclosure of your mental health information in order to provide benefits. I do not participate in any insurance panels. I can prepare a statement or superbill for you to submit to your insurance company to seek any reimbursement you may be eligible for. I would be an out-of-network provider. It is the client's responsibility to verify eligibility, coverage, and benefit levels with your insurer. (See below regarding a federally mandated "good faith estimates" of health care costs.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
- Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy or picture of your Good Faith Estimate.
